ABSTRACT

Objective:To observe the clinical efficacy of autologous platelet rich gel (APG) in the treatment of type 2 diabetic foot (DF) patients and the effect of APG on the expression of metastasis-associated lung adenocarcinoma transcript 1 (MALAT1) in peripheral blood mononuclear cells (PBMCs).Methods:A total of 62 patients with DF admitted to the Affiliated Hospital of Kangda College of Nanjing Medical University from February 2021 to May 2022 were randomly divided into a control group (30 cases) and an observation group (32 cases) using a random number table method. The control group received ultrasound debridement and dressing change treatment, while the observation group received ultrasound debridement combined with APG treatment. After 6 weeks of treatment, the effective rate, transcutaneous oxygen partial pressure (TcPO 2), and serum tumor necrosis factor- α (TNF-α), interleukin-6 (IL-6), vascular endothelial growth factor (VEGF), hypoxia inducible factor α (HIF-1 α)and the level of MALAT1 expression in PBMCs of the two groups of patients were observed. The Pearson correlation analysis was used to investigate the relationship between the expression change of MALAT (△ MALAT1) and the total effective rate of treatment. Results:The total effective rate of the observation group was higher than that of the control group [93.75%(30/32) vs 73.33%(22/30), P<0.05]. After treatment, the systolic blood pressure (SBP), diastolic blood pressure (DBP), cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), fasting blood glucose (FPG), glycosylated hemoglobin (HbA 1c), urinary microalbumin/creatinine (UACR), uric acid (UA), white blood cells (WBC), TNF- α and IL-6 of both groups had decreased compared to before; HIF-1 α, VEGF and MALAT1 increased compared to before treatment (all P<0.05); After treatment, there was a statistically significant difference in UA, HIF-1α, VEGF, and MALAT1 between the observation group and the control group (all P<0.05). Pearson correlation analysis showed that Δ MALAT1 in DF patients was negatively correlated with TNF -α ( r=-0.61, P=0.02), IL-6 ( r=-0.52, P=0.04), WBC ( r=-0.53, P=0.03), and positively correlated with VEGF ( r=0.58, P=0.03) and HIF-1α ( r=0.54, P=0.03). The total effective rate of DF treatment was higher in the high change group of△ MALAT [88.37%(38/43) vs 73.68%(14/19), P<0.05]. There was no statistically significant difference in the incidence of adverse reactions between the two groups ( P>0.05). Conclusions:APG can significantly upregulate the expression of MALAT, improve wound tissue blood perfusion, wound angiogenesis, and inflammatory response, promote ulcer healing, and changes in MALAT expression can help determine the prognosis of DF.

ABSTRACT

As an important tool to evaluate cardiac autonomic function, heart rate variability (HRV) is closely associated with ischemic stroke. The change of HRV can not only increase the incidence and recurrence risk of ischemic stroke, but also affect the severity of the disease and the risk of complications, resulting in poor outcomes. Effective intervention for patients with stroke to restore their autonomic nerve function is expected to become a new target for the treatment of ischemic stroke. This article reviews the research progress of the correlation between HRV changes and ischemic stroke.

ABSTRACT

Objective To explore the mediation role of emotional labor in the relationship between workplace ostracism and emotional exhaustion, aiming to provide practical implications to manage workplace ostracism. Methods Using time-lagged survey, a total of 370 valid responses were collected at two time points from the nurses in a general hospital in Chengdu, including the data of demographics, workplace ostracism, emotional labor and emotional exhaustion. Structural equation model (SEM) was employed to test the relationships. Results The means of key variables were: workplace ostracism 1.30 (1.00, 1.73),surface acting 1.50 (1.00,2.25), deep acting 3.00 (1.33, 4.00), and emotional exhaustion 3.53 (2.60, 4.20). There was a significant positive relationship between workplace ostracism and surface acting (P<0.01), just as well as the relationship with deep acting and emotional exhaustion (P<0.01). Surface acting led to emotional exhaustion of nurses (P<0.01) and mediated the positive relationship between workplace ostracism and emotional exhaustion. Deep acting was not significantly related to emotional exhaustion (P>0.05). SEM analysis demonstrated an acceptable and reasonable model fit. Conclusions The surveyed nurses reported a medium and low level of perceived workplace ostracism and emotional labor, but a high level of emotional exhaustion. Workplace ostracism has a positive effect on nurses' emotional exhaustion via surface acting. Hospital administration should take measure to prevent and intervene in workplace ostracism, provide organizational and supervisory supports to nurses, and particularly enhance nurse's emotional management skills and encourage use of deep acting strategy to minimize the detrimental effects of workplace ostracism.

ABSTRACT

Objective To investigate the role of phosphatidyl inositol 3 kinase (PI3K)/Akt pathway in the protection of focal cerebral ischemia reperfusion injury in rats with limb ischemic postconditioning (LIP) by detecting the expression levels of p-Akt protein, and caspase-9 and Bcl-2 mRNAs after remote LIP. Methods Forty-two Wistar rats were randomly assigned to 3 groups: sham operation, ischemia-reperfusion (IR) and LIP groups. The middle cerebral artery ischemia-reperfusion injury model was induced by the suture method in the IR group and the LIP group. In the LIP group, three circulatory LIP ( 5 min ischemia/5 min reperfusion) in the contralateral femoral artery were performed before reperfusion 2 h after cerebral ischemia. Infarct volume was measured by 2,3,5-triphenyltetrazolium chloride staining. The expression of p-Akt protein was detected by immunohistochemical staining and the expression levels of cystin-9 and Bcl-2 mRNAs were detected by in situ hybridization. Results Compared with the IR group, the infarct volume in the LIP group was significantly reduced ( P<0.05). The expression levels of p-Akt protein and Bcl-2 mRNA significantly increased (all P<0.05), and the expression level of caspase-9 mRNA significantly decreased (P<0.05). Conclusions LIP can reduce the volume of cerebral infarction in focal cerebral ischemia-reperfusion injury in rats. Its mechanism may be involved in up-regulation of p-Akt protein and Bcl-2 mRNA expression and down-regulation of caspase-9 mRNA expression, suggesting that LIP can alleviate cerebral ischemia-reperfusion injury through PI3K/Akt pathway.

ABSTRACT

Glycemic variability is closely related to diabetes complications, and might play a more valuable role in clinical risk assessment. The glycemic variability could effect the prognosis of cardiovascular disease and ischemic stroke.Meanwhile,it has a close relationship with the presence, development and severity of diabetic nephropathy. Moreover,as an independent risk factor for the damage of macro- and microvessels,it has a positive relationship with the severity of target organ. Therefore, the strategy to regulate glycemic variability is a valuable measure of glycemic control, and may be of great significance to strengthen the effect of diabetes treatment and reduce the damage on target organs.

ABSTRACT

Objective To investigate correlation between QT interval(QT),corrected QT interval(QTc) and metabolic syndrome(MS). Methods Residents who participated in our survey concerning atherosclerosis and related diseases conducted in Shenyang were included. They accomplished questionnaire, physical examination, laboratory tests and electrocardiography test. We divided them into MS group and non-metabolic syndrome (NMS)group according to International Diabetes Federation(IDF)diagnostic criteria for MS. QT interval was measured from the standard 12-lead electrocardiogram. QTc was calculated by using Bazett and Fridericia equations. We analyze correlation of QT ,QTc and MS. Results A total of 739 residents who were 35~64 years old were included. Individuals with MS had longer QTcB and QTcF than NMS group[(415.8 ± 31.9)ms vs.(410.1 ± 32.1)ms, (407.2± 29.1)ms vs.(402.6 ± 28.8)ms,P<0.05]. The more the number of abnormal MS parameters they had, the longer the QT,QTcB and QTcF they had. Regression analysis showed that QT was associated with serum potassium, smoking, blood glucose, and LDL,and QTcB and QTcF were associated with hypertension, waist circumference and blood potassium. Conclusions MS is associated with corrected QTc. Careful ECG monitoring among persons with MS for early detection of a long corrected QT interval may prevent severe and often fatal arrhythmias or sudden death.

ABSTRACT

Objective To investigate the relationship between CA199 and clinical indicators and its related factors in patients with type 2 diabetes mellitus(T2DM). Methods A total of 1224 hospitalized T2DM patients were enrolled retrospectively from the Endocrinology and metabolism department of our hospital. All the patients were divided into two groups according to CA199 level:negative group (CA199<37 U/ml) and positive group(CA199≥37 U/ml). Clinical and biochemical data were compared between the two groups. Results The level of FPG,HbA1c,UACR,and the prevalence of diabetic microvascular complications were significantly higher in CA199 positive group than in negative group[(13.12±4.58)vs(10.22±4.01)mmol/L,(10.84±2.58)% vs (8.78±2.22)%,(38.25±3.15) vs (29.09±1.02)mg/g,P<0.05]. Serum albumin level was lower in CA199 positive group than in negative group[(38.59±5.33) vs (40.13±5.63)g/L,P<0.05]. Pearson correlation analysis showed that CA199 was associated with FPG,HbA1c,and serum albumin(r=0.251,0.260,-0.082,P<0.05 or P<0.01). Multiple logistic regression analysis further found that FPG and HbA1c were the independent risk factors for CA199(β=0.793,0.975,P<0.05). Conclusion The elevated CA199 in T2DM patients correlated with poor glucose control and development of diabetic microvascular complications.

ABSTRACT

Objective To explore the possible role of miR-146a in the pathogenesis of diabetic peripheral neuropathy( DPN) by investigating the correlation of miR-146a level in the peripheral blood mononuclear cells (PBMCs)with tumor necrosis factor α(TNF-α), interleukin-6(IL-6), and C reactive protein(CRP). Methods The expression levels of miR-146a in the PBMCs were measured using realtime PCR in 81 patients and 34 healthy individuals. The correlation of miR-146a expression with clinical parameters was analyzed. Results Compared to those in the healthy individuals and patients without DPN, miR-146a level was significantly decreased in diabetic patients with DPN while TNF-α, CRP, IL-6, HbA1C, systolic blood pressure, and homeostasis model assessment of insulin resistance index(HOMA-IR) were significantly increased(all P<0.05). Duration of diabetes, blood uric acid, serum creatinine, and urinary albumin to creatinine ratio in diabetics with DPN were higher compared with diabetics without DPN(all P<0. 05). The expression level of miR-146a was negatively correlated with diabetes duration, HOMA-IR, HbA1C, TNF-α, and IL-6. Multiple linear regression showed that HOMA-IR, HbA1C, TNF-α, and IL-6 were independent factors of miR-146a. Conclusion miR-146a may be involved in the development of DPN through inflammation, glucose metabolism, and insulin resistance.

Results The infarct volumes in the ischemic postconditioning group w ere reduced significantly compared w ith those in the ischemia-reperfusion group (6 h:143.3 ±8.7 mm3 vs.166.8 ±7.5 mm3, t=4.104, P=0.006;12 h:151.7 ±7.8 mm3 vs.171.6 ±9.1 mm3, t=3.314, P=0.016; 24 h: 159.2 ±9.3 mm3 vs.177.1 ± 7.6 mm3, t=3.000, P=0.024;48 h:166.9 ±9.6 mm3 vs.184.9 ±9.0 mm3, t=2.732, P=0.034;72 h:172.0 ±9.1 mm3 vs.198.1 ±8.2 mm3, t=2.640, P=0.039), and the positive cel numbers of BDNF (6 h:23.98 ±4.07 vs.18.63 ±2.5, t=2.479, P=0.038;12 h:27.64 ±3.18 vs.22.01 ±3.14, t=2.817, P=0.023;24 h:34.82 ±4.17 vs.28.46 ±4.05, t=2.446, P=0.040; 48 h:34.30 ±3.27 vs.26.29 ± 3.26, t=3.872, P=0.005;72 h:28.77 ±3.53 vs.23.64 ±3.54, t=2.297, P=0.051) and TrkB (6 h:33.83 ±3.90 vs.21.51 ±3.86, t=5.012, P<0.001; 12 h:38.59 ±4.84 vs.23.41 ±3.67, t=5.586, P<0.001;24 h:46.07 ±3.06 vs.28.78 ±3.61, t=8.169, P<0.001; 48 h:47.90 ±3.30 vs.29.51 ± 3.81, t=8.160, P<0.001; 72 h:42.78 ±4.07 vs.27.46 ±3.19, t=6.623, P<0.001) per high-pow er field at each time point in the ischemic postconditioning group w ere significantly more than those in the ischemia-reperfusion group. Conclusions Ischemic postconditioning upregulates the expressions levels of BDNF and TrkB proteins after ischemia-reperfusion and reduces cerebral infarct volumes. BDNF/TrkB may play an important neuroprotective effect in ischemic postconditioning.

ABSTRACT

Objective To investigate the association between serum uric acid (SUA ) level and diabetic peripheral neuropathy (DPN ) in middle‐aged and elderly population. Methods A total of 207 middle‐aged and elderly patients with T2DM were enrolled in this study. All the subjects were divided into two groups:T2DM only group and DPN group. The DPN group was further divided into three subgroups (mild ,moderate and severe group) according to TCSS score. Pearson correlation analysis was used to evaluate each index ,and binary logistic regression was used for statistical analysis of risk factors for DPN.Results SBP ,UACR ,BUN ,Scr ,TC ,LDL‐C and SUA were significantly higher in DPN group than in T2DM group. SUA were significantly increased together with the increase of the severity of DPN [(279.66 ± 56.53)vs(308.93 ± 52.96)vs(330.24 ± 48.79) μmol/L](P<0.05). Conclusion SUA level is an risk factor for the development of DPN in the middle‐aged and elders with T2DM. SUA is positively correlated with the severity of DPN.
